Targeted Case Management

MAS Community Health offers a family-centered community-based mental and behavioral health case management service for children with an identified mental or behavioral health condition, or for young children at risk of developing one, who are in need of specialized services.

Our team of highly trained Case Managers will work collaboratively with children and their families to identify their unique needs while providing coordination and navigation through the complex mental health services system.

• Coordinating with school supports such as IEP’s, 504 Plans, and Team Meetings
• Assisting with complex form completion and submission
• Referral coordination for med management, outpatient therapy, or skills services
